Single Day Camp Registration

Health & Fitness / Medical Authorization. I represent that the Participant is in good physical condition and capable of participating safely. I understand it is my responsibility to consult a physician if I have any questions about fitness to participate. In the event of an emergency, I authorize SCF/GGAA Inc. and its representatives to obtain or provide emergency medical care for the Participant (including first aid, CPR, and transportation to medical facilities). I agree to be responsible for all costs related to medical care and transportation, whether or not I have insurance.

Release of Liability (SCF/GGAA Inc.). To the fullest extent permitted by law, I release and discharge SCF and GGAA Inc., and each of their respective directors, officers, employees, volunteers, agents, representatives, independent contractors, coaches, staff, sponsors, partners, successors, and assigns (collectively, the “Released Parties”) from any and all claims, demands, causes of action, damages, losses, liabilities, costs, and expenses (including attorney’s fees) arising out of or related to the Participant’s participation in the Activity, including claims for personal injury, death, or property damage, whether caused by negligence or otherwise, except to the extent caused by the gross negligence or willful misconduct of a Released Party where such limitation is not permitted by law.

Release of Liability (Facilities / Premises). I understand that SCF activities may occur at gyms, schools, parks, private facilities, and other locations not owned or controlled by SCF/GGAA Inc. I release any such facility owners, operators, landlords, lessors, and their respective directors, officers, employees, and agents from claims arising out of or related to participation in the Activity, to the fullest extent permitted by law, except to the extent caused by their gross negligence or willful misconduct where such limitation is not permitted by law.

Property / Equipment. I understand that I am responsible for my own property and the Participant’s property. SCF/GGAA Inc. is not responsible for lost, stolen, or damaged personal property.

Participant Conduct / Safety Rules. I agree that the Participant will follow all instructions, rules, and safety guidelines provided by coaches, staff, or facility personnel. I understand that SCF/GGAA Inc. may remove any participant from activities for unsafe, disruptive, or inappropriate behavior, without refund.

Indemnification. I agree to indemnify, defend, and hold harmless the Released Parties from and against any claims, liabilities, damages, losses, and expenses (including attorney’s fees) arising from (a) my or the Participant’s participation in the Activity, (b) any breach of this Agreement, or (c) my or the Participant’s actions or omissions, including claims brought by third parties.

No Representations. No express or implied representations have been made on behalf of SCF or GGAA Inc. other than those contained in this Agreement.

Severability. If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions will continue in full force and effect.

Governing Law; Venue. This Agreement will be governed by the laws of the State of California. Any dispute arising out of or related to this Agreement or the Activity will be brought in a court of competent jurisdiction in Los Angeles County, California, unless otherwise required by law.
